I with drew a lump sum from my IRA and then returned it within the 60 day limit but it crossed over - withdraw in Dec 2021, return in Jan 2022, still within the 30 days. How do I report this?

It doesn't matter that the dates span two different years. The question you will be asked is if you rolled it over within the 60 day time period. If you did so, then answer yes.
