I've paid extra for social security tax. I include that in the filed tax return form but I haven't received the refund yet. Do you know if there is anything wrong?

I think turbo tax included that number that I should get refund in schedule3  line 11. That number is not added to schedule1 line 21.a though. Could that be the issue? Should I file an amend?

If it was on line 18d it was included in the refund on line 21.  You don't get a separate refund check for it.  Print out the 1040 and follow the math. It either increases your refund or reduced a tax due.

I e-filed the form. Actually I've received the federal refund specified in line 21a of 1040 in March. But the social security tax was not refunded after 3 months.

And in 1040 line 21a that amount (which I've already received 3 months ago) does not include the social security tax that should be refunded, as listed in 18d. Is that expected?

The amount of SS excess payments would be entered on the Form 1040 Line 18d.  Lines 17 thru 18e are added together and the result is on Line 19.

If line 19 is more than line 16, subtract line 16 from line 19.  This is the amount you overpaid.  And is entered on Line 20.  Form 1040 Line 20 is your federal tax refund so the excess SS taxes withheld will be included in Line 20.

You had more than 1 employer?  And the total SS in box 4 is more than $8,239.80?  You get the excess back.  Check 1040 Schedule 3 line 11 for it.  Then Schedule 3 goes to 1040 line 18d.

That won't affect when you get your refund.  The IRS is slow this year because of Covid-19.  Did you efile or mail your return? They just started processing mailed returns that have been stacking up..

If you efiled....Was your return Accepted? First double check your efile status here
https://turbotax.intuit.com/tax-tools/efile-status-lookup/

When you efile you get back 2 emails. The first email only confirms the transmission. The second email says if the IRS (or state) Accepted or Rejected your efile.

Then only the IRS can tell you. IRS Where's my refund https://www.irs.gov/refunds/
Make sure you are using the refund amount from your actual federal tax return and not the combined amount Turbo Tax showed you at the end. That was for federal and state combined.

When you log into your account you should also see the status and if it was Accepted or Rejected, Started, Printed, Ready to Mail, etc.  what does it say?

When will you get your refund,
