I used my W-2 that was posted on turbo tax through my employer. They never sent me a copy. Now I find out 401k & Health was not deducted. Anyone know why?

Deducted from what?  From your checks all year?  Are they on the W2 box 12?  They are not deductions on your tax return.  They are pre-tax meaning they come out of your pay before taxes and are not included in the Wages in box 1 so you can't deduct them again.  Notice that box 1 is less than boxes 3 & 5.
