Repayment of a 401(k) loan is not a rollover.
Did you receive a Form 1099-R related to the amount of the loan? If so, perhaps you received a plan loan offset distribution from the original 401(k) and depositing that into another traditional qualified retirement account would be a rollover.
If receive no Form 1099-R then it's not a rollover, it's simply a loan repayment. With no Form 1099-R, there is nothing to report regarding the loan.
