If the distribution was NOT a loan, then the payer of the distribution is required by law to provide you a Form 1099-R for the distribution made in 2019.
Contact the payer and request the Form 1099-R. If they refuse to provide the 1099-R the follow the steps in the this IRS Tax Topic - https://www.irs.gov/taxtopics/tc154
Thanks for the reply. Is the payer considered the company I worked for or Morgan Stanley who issued the check? @DoninGA
It would be Morgan Stanley since they are the payer and probably the administrator of the 401(k) account.
