Better call the plan. They think it was a distribution and not a loan. Or you didn’t pay it back on time so it became a distribution. Do you still work there?
As VolvoGirl indicates, code 1 in box 7 of a Form 1099-R indicates a distribution, not a loan. As such, it's not clear how you could be paying it back, particularly beyond 60 days from the date of the distribution. Further, the code 1 would likely suggest that this was a hardship distribution if you are still employed by the company that provides this 401(k) plan (and I'm not sure how you could be returning any of this back to the plan if you were not still employed with this company, and hardship distributions are not eligible for rollover, so something is definitely not right this.
