I took a 401k early withdrawal in 2025 due to domestic violence and was told it was exempt from the 10% penalty. How do I report that on my 1099 R income on TurboTax?

Was told by employer and Fidelity that this was a qualified domestic violence withdrawal thru Secure 2.0 distribution rules

Look on the Income page for Retirement Income, there is a section for 1099-R forms.  After entering the information, you will be asked if you qualify for any of the exceptions, you can check the box for DV.  Be aware that the penalty exclusion is on 50% of the withdrawal, or $10,000, whichever is less.  It is not a 100% exclusion of the penalty.

Yes, when you have an early withdrawal of a 401(k) when you are a victim of domestic abuse, you are exempt from the 10% additional tax.  When you enter your Form 1099-R, the program will guide you through more questions to qualify for the exception.  To enter your form 1099-R:

1. Open or continue your return.
2. In the Federal section, select Wages & Income.
3. Scroll to locate Retirement Plans and Social Security.
4. Select Start or Revisit next to IRA, 401(k), Pension Plan Withdrawals (1099-R).
