I selected mail my return and i received the 8960 form, but it does not contain a sinature page as the 1040 does. What am I to do to ad my name and bank information

What do I do. The printed out return did not include a signature page. Do I fill out a 1040 form and attach it to the8960?

You attach the Form 8960 to the federal tax return Form 1040.  You sign and date the Form 1040 on page 2.
