Please clarify.
Check was date 2021 and received no tax documents , no 1099R although they stated they will send one out...can I create my own? the bottom of the ck has most information but had to call for Payer ID ...they already deducted taxes as ordinary income so how do I show taxes paid ?
The 1099-R has a box for federal taxes withheld, state also. See Form 1099-R PDF. If you are being sent a 1099-R, it would be best to wait for it in case there is a discrepancy. It may be available online more quickly than mailing. If you want to make your own, there are rules. You can let the JP Morgan know that you will follow through and report them. A substitute 1099-R is form 4852, also used for substitute w2s.
The form 4852 is a last resort. This is what the form says:
If you don’t receive the missing or corrected form from your employer or payer by the end of February, you may call the IRS at 800-829-1040 for assistance. You must provide your name, address (including ZIP code), phone number, social security number, and dates of employment. You must also provide your employer’s or payer’s name, address (including ZIP code), and phone number. The IRS will contact your employer or payer and request the missing form.
Because of this, you cannot e-file a return with a 4852, it must be mailed in.
