I received a full distribution from my IRA / annuity early (48 yrs old). Transamerica in Iowa sent the 1099-R. I live overseas. Is this income considered from Iowa?

Only if Iowa happens to be your state of residence, or, Transamerica reported the transaction to Iowa in boxes 12-14 of your 1099-R.

The fact that Transamerica is located in Iowa is irrelevant.
