I pulled money out of my 401k plan but I am currently paying myself back as I work? Would I need a 1099 form?

It sounds like that was a 401k loan, in which case you would not need a 1099-R since it's a non-taxable event.

If you took a distribution (i.e. no payment plan was put in place), then you would need to report your 1099-R.
