Yes, you should receive a Form 1099-R for each distribution that took place during the tax year. 

The financial institutions have until January 31st to mail these forms to you, so they may still be on the way.  If you do not receive them within a few more days, contact the issuer(s) to request your forms.

Yes, you should receive a Form 1099-R for each distribution that took place during the tax year. 

The financial institutions have until January 31st to mail these forms to you, so they may still be on the way.  If you do not receive them within a few more days, contact the issuer(s) to request your forms.

I went on website where I had my Roth IRA and 401k and only 1099 form it had was for 401k.

There may be some very specific cases where a 1099-R would not be issued, or it may not be issued during the current tax year.  For an ordinary distribution though, you should be getting a 1099-R for the Roth as well as the 401k.  If you don't get it soon, check with the financial institution directly.
