Was this done in 2019 and you have a 2019 Form 1099-R reporting this distribution?

If so, what is the code in box 7 of the Form 1099-R?

Does box 2a show the correct taxable amount?

For an In-plan Roth Rollover or a direct rollover to a Roth IRA, the Form 1099-R should have code G and box 2a should show the taxable amount.
