the   state   tax  withheld is n  box  10  on  this  form,   but  the  same info is in  box  12  on  other  forms

When you enter the information, enter the information according to what the box is supposed to contain, rather than by the box number.

The form you have is non-standard, but it is clear that you understand what needs to be done.
