I have a 1099-R filled out correctly, but it says I need to put something in box 12 (state tax withheld) when it is left blank. anyone know what this means?

It's ok if no amount appears in box 12 on your Form 1099-R.
When box 12 is empty then no income was withheld from your retirement income.
All states do not tax retirement income. If Form 1099-R, box 12 is empty then leave it blank.
Two additional states that only tax dividends and interest:
New Hampshire and Tennessee
Seven states that do not tax retirement (and other individual income) are:
Alaska, Florida, Nevada, South Dakota, Texas, Washington, and Wyoming.
When box 12 is empty then no income was withheld from your retirement income.
