I have 628.42 listed in box 5 on my 1099-R does that mean my Pension Adim. contributed to my health ins. I don't have a roth IRA

No - Box 5 represents the  employee's contribution to the retirement plan during your career.  It is just for information.  TurboTax uses the amount in box 2a as the taxable amount (unless box 2a is blank and box 2b Taxable amount not determined is marked).  However, since an amount is shown in box 5, box 2a should have the correct taxable amount.
