I had taken out a $1,500 loan out of my 401k and I have paid it back. Do I need a 1099-R form from my financial investment bank to have when I file my taxes? Do I have to pay taxes on that?

A loan from a 401(k) account is not reported on a tax return.

and the repayment is not re[portable and any interest you paid is not deductible
