I donot receive a W-2 for my retirement just a 1099R, in which block 5 lists3372.48 for Insurance premiums. Can I deduct this amount on my OHIO State Income Tax?

If those are health insurance premiums then they are deductible as an itemized medical expense but subject to the 7.5% of AGI haircut. When entering the 1099-R information you are asked if any insurance premiums are deductible on Schedule A and you can enter the amount deductible there. TT/Ohio will pick up that medical deduction from there.
