I don't know if I ever had a Lump sum distribution from social security, if I did it would have been around 2004 0r 2005...

I no longer have those records. I thought I only had to keep records for 7 years...

If you did not receive a lump sum on your 2016 SSA-1099 then answer the question concerning lump sums as NO.
