I did not receive a form 1099R for a distribution from my 401K. Do I need one?

If you took money out of a 401k in 2020 you do need a 1099R. Did you take money out? If so you need to contact the custodian of the account and get a 1099R for it.

Yes. If you took any money out of a retirement plan in 2020, this is reportable. You need to contact the company that manages your 401(k) and request the form.
