The amount contributed to a retirement account at work is reducing the wages in Box 1 of the W-2, if it a Traditional Retirement Account (non-Roth).
The contributions at work do not need to be accounted for as they are reflected already when the W-2 is entered.
You need to report the 1099R for the distribution only (as the contributions are already reported).
