You get an SSA-1099 for disability, not a W-2. Go to the Social Security website for it.
If your only income is SSI or Social Security then you have nothing to enter on a tax return (and no reason to even file a return ) unless you had tax withheld or have a 1095A for Marketplace insurance or for 2020 & 2021 you have missing stimulus payments to get as a credit.
HOW TO GET SSA-1099
If you cannot find your SSA1099,you can get one from your nearest Social Security office, or there is a way to go on the Social Security website and request one. Go to www.ssa.gov You have to create an account with a user name and password.

Go to Federal> Wages & Income>>Retirement Plans and Social Security (SSA1099 and 1099RRB) to enter your SSA1099.
