I am going to make a $5000.00 contribution to my IRA before April 15. Is there a form that I need to fill out and do I have to fill it out before I file?

There is no special form. Just enter the contribution amount into the program. This will insure, prior to the investment, that you can deduct for the Traditional or contribute to Roth,  the amount you want to. Just make sure you actually put the money into the IRA before the deadline.
