Was that interest on the death benefit? Enter it as interest in Federal >Wages and Income >Interest.

How to I file a death benefit with $7.89 taxable and no 1099-R

No information on what the $7.89 was. Filed a substitute form. If the taxable was $10.00 or over would have gotten a 1099-R. Seems a stupid money saving rule by New York Life Annuities. Paperwork to be filled out was horrible.
