How to file returns with a 401k roll over to Roth IRA. I only see a form 5498 from Fidelity?

Please enter your 1099-R for the 401k rollover and answer all follow-up questions carefully:

Click on "Search" on the top right and type “1099-R”
Click on “Jump to 1099-R”

You do not enter Form 5498, please keep it for your records.
