My husband and I are both 64, I work, he's retired. In 2018, I contributed $15,688 to my 401(k). Our combined income is $91,076 (he gets Social Security) and combined taxable income is $63,701. Is the total we can contribute to an IRA $6,500? Or can we contribute $5,500 plus $1,000 each year over 50 years old (or $19,500)? Neither of us took any payouts from our 401(k)'s.

Form 2019 and 2020 you can contribute $7,000. That is a combination of the regular limit of $6,000 and the $1,000 catch up contribution if you are 50 or older.
