How do I report my 401K rollover of pretax funds to a traditional IRA and the after tax funds to a Roth IRA when I only received a single 1099-R from Fidelity.

The single 1099-R I received correctly reports the gross distribution in box 1 and the after tax amount in box 5. The Distribution code in box 7 is G.

To accommodate TurboTax, you must split the Form 1099-R into two, one for the after-tax portion rolled over to the Roth IRA and another for the pre-tax portion rolled over to the traditional IRA.  The dollar amounts on the split forms should total the amounts in the corresponding box of the original.
