How do I get a substitute W-3 tax form?

You should first contact the employer that you worked for and issued you the W-2. They should be able to provide you with a copy. If you cannot get a copy from your employer, you can get a wage and income transcript, containing the Federal tax information your employer reported to the Social Security Administration (SSA), by visiting the IRS Get Transcript page.

If you e-filed your tax return or you didn't attach your Form W-2 to your paper return, then use one of the transcript options above. Otherwise, you'll need to contact your employer or Social Security Administration for a copy.
