How do I document a retirement plan 60-day rollover that spanned from a late December 2018 withdrawal to February 2019 for the rollover deposit?

In late December 2018 I took a withdrawal from my 403B retirement account. I redeposited it (rolled over) within 60 days with the same institution but in an IRA. The rollover deposit occurred in February 2019 and within the 60 day rollover period. The financial institution will not issue a revised 2018 1099R. How do I document the rollover so I don't get stuck with a tax bill for the withdrawal in 2018?

keep your paperwork showing the deposit into the IRA with your tax records.

You will need it only if the IRS asks you to show it

How do I document a retirement plan 60-day rollover that spanned from a late December 2018 withdrawal to February 2019 for the rollover deposit?

The rollover contribution will be recorded on your 2019 Form 5498.  As fanfare said, just make sure to retain your 403(b) and IRA account transaction statements to be able to substantiate that the rollover was completed by the deadline, in case the IRS questions whether or not the rollover was completed.
