How do I answer "on the Form 1099-R you received, is the IRA/SEP/SIMPLE box between box 7 and box 8 checked?" Not checked OR blank, there's no such box!!

Did you receive a Form 1099-R or some other form for the withdrawal?  See this IRS website for a Form 1099-R and notice there is a box labeled IRA/SEP/SIMPLE to the right of box 7 - <a rel="nofollow" target="_blank" href="https://www.irs.gov/pub/irs-pdf/f1099r.pdf">https://www.irs.gov/pub/irs-pdf/f1099r.pdf</a>

Thanks.  I looked at the form you sent, and I see the IRA/SEP/SIMPLE box but I don't have anything like that on my 1099-R form.  There's no withdrawal involved; this is a form showing military retirement pay.  I appreciate the help.

The military retirement/pension Form 1099-R provided does not always follow the approved IRS form.  With that being said just answer the question concerning the IRA/SEP/SIMPLE box being checked as No.
