It's just another 1099-R form...but when you fill out the new 1099-R form, there is a checkbox between your box 7 and box 8 entries that says "IRA/SEP/SIMPLE" next to it...you do NOT check that box for the Michigan pension since it is not an IRA...and then the $$ will end up on a different line in the federal tax return.
(that box is checked for the 1099-R for the IRA distribution)
