Form 1099-R box 2B, has 2 parts, taxable amount not determined & total distribution, both say NO. Should I check 1 or both of these boxes in TT or leave them unchecked?

If they say no on your 1099-R leave them unchecked.  This is especially the case if you have an amount in box 2a (taxable income).  The box taxable income unknown doesn't apply:  the taxable income is being reported.

If you're looking at a document for an IRA, taxable amount is not determined.
See IRA box in the middle of the form.
