For the traditional IRA contributions do I have to enter the amount that was taken out of my paycheck every pay period?

IRA contributions are not taken from a paycheck. Employer retirement plan contributions are, and those are reported on the W-2 box 12 and nowhere else. They are NOT IRA contributions.
