Do you wait for a form from your 401k or do they even require anything?

That depends on whether you put money into the 401(k) plan or took money out of the plan. Contributions to your 401(k) plan are shown in box 12 of your W-2 with code D (or AA for a Roth plan). If you took money out of your 401(k) plan, you should wait until you receive the Form 1099-R that reports the distribution before filing your tax return.
