If you have a 1099R you enter it.

 

 

To enter your retirement income, Go to  Federal> Wages and Income>Retirement Plans and Social Security>IRA  401 k) Pension Plan Withdrawals to enter your 1099R.

Yes you have to report all 1099R you get.  What code is in box 7?  Code G is a direct transfer and not taxable.  It will say Rollover by 1040 line 5b.
