I changed my job in 2016 and had 401K (previous company) direct rollover to traditional IRA.  But I missed this information on my 2016 tax return. Do I have to  amend 2016 tax return and file the amended one by mail?

Yes.

You have to do an amendment on 1040X and mail it in.
