Did not receive a 1099, only final Statement of Account for the HSA. It was my wife's through her employer, The State of Alabama. They said they don't give a 1099. ??

If your wife took distributions from her HSA funds to pay for medical expenses, a 1099-SA form is required to be sent. Contact the HSA custodian (not the employer) or the form.
