Can I just change the 1099-R to reflect a distribution instead of an IRA rollover for a $141.00 check that I mistakenly cashed from a 401k settlement fund?

I mistakenly cashed the 401k settlement check instead of rolling it over into my IRA. I'd like to make changes on the 1099-R to reflect a distribution instead of a rollover and also indicate it as such in Turbotax. Is this acceptable?

No.  You should not change the information on the 1099-R.  It will be a red flag to the IRS since they also get a copy of the form.

Contact the issuer of the 1099-R to get a revised form.
