Who sends you the disability income? If you mean you are getting Social Security disability then they mailed you a SSA1099 in January. If you misplaced it you can get a copy from Social Security.
There is a way to go on the Social Security website and request one. Go to www.ssa.gov You have to create an account with a user name and password.

If your disability is coming from some other source then you should have received either a W-2 or a 1099 for it by the end of January/early February. You would have to request a copy from the company that sends you the disability income if you cannot find the one that was sent to you.

Go to Federal> Wages & Income>>Retirement Plans and Social Security (SSA1099 and 1099RRB) to enter your SSA1099.
Some disability is not taxable ( like VA benefits) so no 1099-R is ever issued.
