I cashed out my 401k after leaving the company I worked at for nearly 5 years.
Why did I receive 2 1099 Forms in the mail?
It is showing tax withheld on the 1099 form with the larger amount listed, yet showing all zeros on the form with the smaller amount.
I'm confused because I only received the one time payout of the larger amount.
Any idea why I received 2 forms? And should I include both of them?

What is the code (or codes) in box 7 of each of these Forms 1099-R?
Does either form have a nonzero amount in box 5?
You should contact the 1099-R issuer for additional information.
One might be a correction of the other, or the disbursement method might have different categories. You might also have had different accounts within the same company. This is just general information.
