1. Federal Taxes
2. Wages and Income
3. Scroll down to Retirement Plans and Social Security
4. Select IRA, 401(k), Pension Plans (1099-R)

If you have already filed and your return has been accepted -and just now received the form- you will have to amend your tax return;the return is mailed; takes about 16 weeks to process. Here are amending instructions.
