Question

Since box 5 on your CSA-1099-R can hold more than one item, tell us how much of your box 5 is for health insurance. How do I know? Where is box 5 information explained?

TT is asking for me to put in Health Insurance Amount from box 5 that contains more than just health insurance. Where would I find out how much is just for health insurance vs Employee Contributions/Designted Roth Contributions?

If you can not determine how much of your Box 5 is for health insurance, you can call OPM to ask them.  You can contact OPM as follows: To contact the Office of Personnel Management (OPM), you can call their Retirement Services at 1-888-767-6738 (Monday-Friday, 7:40 AM to 5:00 PM ET), email them at retire@opm.gov, or visit their website by clicking here

If you can find the annuity statement that OPM sent you for 2024, it lists the "Monthly Health Benefits" that are deducted from your pension. Multiply that times 12, and that is the amount of Box 5 that was for health insurance.

Thanks for this response.  You says "If you can find the annuity statement that OPM sent you for 2024, it lists the "Monthly Health Benefits" that are deducted from your pension. Multiply that times 12, and that is the amount of Box 5 that was for health insurance."  Well, I retired last April, and I have the annuity statement that OPM sent me for 2025.  But it only shows the health benefits for Oct-Dec, excluding the interim and catch up annuity payment covering May-Sept 2025.  How should I handle this?  Multiple the Monthly Health Benefits by 12?  By 8 for the months I was retired and my annuity paid the health care benefits?  Or only by the 3 months included in my 2025 summary of payment?  Thanks for any help you can provide!

Go to the OPM site where you downloaded your 1099-R.

On the left side is a list under

     "My Annuity"

    Select "Annuity Statements"

The screen will show your most recent payment and lists the breakdown (amount) attributed to your health insurance.

Near the top right of the screen, find the drop-down for "Payment Dated" and switch to the payments made in 2025 adding the health insurance payments for each month.

That should give you your 2025 total.
