So this is a stock program with my work. Does my company name go there or the county the company headquarters are in? My case would be Germany. Sorry for all the questions.

On the Form 1099-DIV you received - There is No PAYER’S name, street address, city or town, state or province, country, ZIP or foreign postal code, and telephone no. ?

Is there a PAYER’S TIN entered which would be nine digits, ie 12-3456789?
