My W-2 Box 12 contains the following: "D 21 1783.61" is the 21 a code? 1783.61 seems too low for 401k withholding but 21183.61 exceeds my total income.

For some reason only known to the company that provides the W-2 they added 21 to the D code to indicate it was for tax year 2021.  So disregard the 21 and only enter the code D and the amount of $1783.61

I am a reservist who was mobilized in 2020 and 2021. Upon my return from mobilization, I exercised my USERRA rights and made "make up" contributions to my 501k during 2022. I have three entries for my civilian W-2 box 12, which say D 20 250, D21 6800, and D 27000. TurboTax reports the D 20 and D 21 entries as duplicate entries and an error.  I need assistance this and do I need to amend my 2020 and 2021 tax returns.

No, you do not need to amend your past year returns.  The make up contributions are excluded from current year taxable income (W2 box 1) already.  In TurboTax you can combine all three code D entries as one code D total.  This code D is informational only for you, as the W2 has already been sent to the IRS and Social Security Administration by your employer. Generally, the boxes 1 and 2 are the boxes that need to be reconciled on your tax returns.
