Question

I transferred money from a 401K to a traditional IRA, then withdrew those funds. Should I have 2 1099R forms?

Yes. Form 1099-R is used to report the distribution (including rollovers) of retirement benefits such as pensions, annuities or other retirement plans. Presumably, you would receive one Form 1099-R related to the rollover (transfer) and one Form 1099-R related to the distribution (withdrawal).
