I have a 1099-R issued in 2009 but it was never reported or claimed in any of my previous return, can I include it it 2018 return?

No, it cannot be included on your 2018 tax return.

In general, the statute of limitations for the IRS to assess income tax or for you to claim a refund has expired long ago.

What is the code in box 7 of the Form 1099-R?
Is the IRA/SEP/SIMPLE box marked?
Did you roll any of the distribution over to another retirement account?
