I forgot to add 1099-R information for my wife's pension distribution rollover to a 401k plan, do I need to amend my return even if I own no additional tax?

Yes, once your original return has been processed, you'll need to file an amended return, even though there's no additional tax owed.  The IRS needs this to be reported.
