I did a rollover in 2019 and received the check and a 1099-R form for it. But I misplaced the check, so the actual rollover didn't occur until 2020. Do I file the 1099-R?

I received the first rollover check in 2019, but I misplaced the check and had to void it and request a new one. But that second check wasn't issued until 2020, and so the rollover actually happened in 2020 instead of 2019.

If you received a Form 1099-R, you should report it on your tax return.  It's the same rollover.
