I am not sure I need to submit a Form 5498 along with the 1099-R form.
I withdrew all my contributions from my IRA in 2016. Do I need to submit a Form 5498 along wth the1099-R form?
There's no reporting requirement on a 5498 form. It summarizes amounts going into an IRA. It's an information form for your records.
