Is my HSA included in my 401k maximum?

You should go back to your W-2 and verify your entries.

The amount contributed to a Health Savings Account (HSA) should have no bearing on your maximum 401k contributions.

Amounts reported in box 12 (any lines of box 12) with code D indicate 401k contributions.  Amounts reported in box 12 with code W indicate HSA contributions.  Be sure that your entries match your actual W-2 and that code D was not entered for more than one line of box 12.
